Immunization Tracking System – For Returning Students ONLY
The Immunization Tracking System is a web-based portal that manages any type of data or compliance requirements that your school may require. Our staff will work with you closely via e-mail to make sure that your account is up to date and that all the required documentation has been sent to our office.
- Download Clinical Requirment Instructions and General Release Form
First start by downloading the required Immunization Forms. You can find these forms underneath the "Forms to Download" section on the right. Please follow the directions found on each form. If you are not sure what forms to download, please consult with your Clinical Compliance Officer.
- Manage Your Immunization Records
All immunization and health care records will require you to scan and upload the corresponding form completed by your healthcare provider and/or documented proof into your ITS Profile. Or you may Fax to 303-339-7521 or Toll Free Fax at 1-877-619-4139.
*PLEASE NOTE: You will need to keep a copy of your completed Immunization Forms as you will use it to input the information into the Immunization Tracking System.
In addition to uploading your documents, it is your responsibility to enter the dates you received your vaccinations as well as the dates that each form was signed by your provider, in the appropriate text boxes.
American DataBank will not enter the information for you. Also, after entering your information please be sure to both 'Save' and 'Confirm' your record, otherwise the information will not be applied to your ADB Profile.
